Birthday Wish Application
Parents please fill out the following form. Parties will be selected on a first come, first served basis. We have slots for 5 children’s parties at each location. The time for the party will be the last Sunday of every month from 4pm-6pm, 5pm-7pm or 6pm-8pm. It will take place at Southlake, Bedford, Hudson Oaks, Arlington, Mansfield, or North Fort Worth.
Please read entire rules below.
- Registration will only be open the 15th to the 5th of the month for the following month’s birthday parties. (EX: If your child has a birthday in December you would be able to register from November 15th thru December 5th). No early registrations will be allowed.
- Child must be a current foster care placement or ADOPTED (PMC, kinship and fictive kin do not qualify according to Urban Air requirements).
- Your family is available to have the party on the last Sunday of the month from 4pm-6pm, 5pm-7pm or 6pm-8pm.
- The party is for the Ultimate Birthday Party Package at a table. The option to upgrade to a room is available for an out-of-pocket cost.
- One Birthday Wish party allowed for each foster/adoptive child.
- For foster children; ages 3 and up
- Adoptive children; ages 3-12.
- Your foster/adoptive child can invite 9 guests of their choice (17 and under) for no charge. (no extra guests are allowed except parents/relatives). Add more jumpers for an additional cost per participant.
- Pizza is a PAID option. Let your Urban Air party rep know if you desire to order Pizzas.
- Each child MUST have a parent sign a jump waiver upon arrival to Urban Air.
- Urban Air will contact the family by phone or email to make party arrangements.
